I agree to forgo, and expressly waive, any damage rights I or my heirs may have against Chicago Swing Music Festival NFP, it's officers, directors, committee members and volunteers or its agents for any and all injury and damages I may suffer by participating in the workshops or dances at any location during the Windy City Lindy Exchange 12 (WCLX 12), held October 1-3, 2010.
I hereby give permission to the Chicago Swing Music Festival NFP, it's officers, directors, committee members and volunteers to take and use images of myself, captured through video, photo, and digital camera during the Windy City Lindy Exchange 12 (WCLX 12) held October 1-3, 2010. I give them permission to use the images for video distribution, promotional material, and any other ways the organization sees fit. I waive any rights I or my heirs may have to compensation, any other rights to the footage.
